Please read the following information carefully.

Dear Students!

A special form for collecting documents confirming a valid reason for missing classes/control elements has appeared on the program website for your convenience.

A medical certificate/other document confirming a valid reason for absence can be submitted through this form.

The medical certificate or a scanned copy must be presented to the study office via this form, by corporate mail, or in person on the day when the student is instructed to return to the classes in accordance with the certificate.

Other valid excused absences are documented within 3 study days.
