Formation of chronic diseases management programs on the example of patients with chronic kidney disease

<p>Nowadays the chronic diseases pandemic &nbsp;is a global world community problem. It has not only medical, but also huge social and economic value. Chronic diseases annually carry away million lives, lead to the heavy complications connected with disability and expensive treatment. Modern health systems are faced by a task to prolong the period of active life, to increase its quality, to keep working capacity at patients with these diseases. In many countries of the world special chronic diseases management programs are developed.</p><p>In our country &nbsp;the chronic disease management programs are developed for diabetes, a hypertensive disease, bronchial asthma, a chronic obstructive lungs disease. The chronic kidney disease unfairly isn&#39;t present in this list, abroad this disease is treated within specially created programs on an equal basis with listed above.</p><p>&nbsp;The chronic &nbsp;kidney &nbsp;disease affects 10-15% of population, without the correct treatment it &nbsp;leads to &nbsp;fast patients disability.</p><p>The &nbsp;master thesis purpose is to develop and prove the organizational mechanism of chronic diseases management (CDM) programs formation directed on increase of medico-economic resources.efficiency.</p><p>According to this purpose in dissertation research the following tasks are set:</p><p>- to reveal the main tendencies in development of the CDM &nbsp;foreign programs;</p><p>- to find out, these programs are how effective, realization leads them to what results;</p><p>- to develop the principles and methodical approaches to implementation of the CDM &nbsp;program &nbsp;in the Russian Federation on the example of patients with a chronic illness of kidneys.</p><p>Object of dissertation research is the health system of the Russian Federation, a state policy of management of health care development.&nbsp;</p><p>Object of research are the mechanism of CDM programs formation: the example of patients with a chronic kidney disease and set of operating influences on increase of health care medico-economic efficiency.</p><p>Theoretical and methodological basis of research. For the solution of objectives the following methods of research were used:</p><p>- The analysis of domestic and foreign literature on a subject of chronic diseases management;</p><p>- Creation of model of management by a chronic kidney disease in Russia.</p><p>Conclusions</p><p>- The CDM programs &ndash; first of all a product of coordination and integration of various divisions in health system at all levels; for their creation it is necessary to enter new funding mechanisms, to train employees in interaction, to organize the new coordination divisions, the integral element - increase of consciousness of patients, formation at them careful attitude to the health;</p><p>- The CDM programs lead to improvement of health in population. In process of development of the CDM programs the number of their participants grows. As a result two multidirectional tendencies face: a) specific costs of one patient are reduced, b) the number of patients grows. Their cumulative action sometimes gives economy, sometimes on the contrary &ndash; growth of expenses. In our country due to absence of sound base introduction of the CDM programs has to have more expensive character in comparison with foreign experience.</p><p>- The program of management of CKD in Russia has to include without fail population screening for identification of patients at the initial stage of injury of kidneys, creation of uniform recommendations at all levels on maintaining patients (more precisely, introduction of the national management, is possible, in a short form, in daily use), creation of bodies of coordination of various interdepartmental structures, tracking of a way of each patient for the maximum extension of the period before hemodialisys.</p>
