Perinatal palliative care is a new direction of modern medicine, developing at the intersection of obstetrics-gynecology, neonatology, pediatrics and palliative care for children. This work is devoted to the development of approaches to the introduction of palliative care in perinatal medicine in the Russian Federation. The study investigated aspects of perinatal palliative care as a reproductive choice for incurable fetal malformations. The foreign experience in the development of this direction is considered, foreign methods of implementing international programs are highlighted. Practical parts of the work based on the performance indicators of the National Medical Research Center for Obstetrics, Gynecology and Perinatology of the Academician V.I. Kulakov of the Ministry of Health of the Russian Federation and statistical indicators of the country, revealed the grounds for this assistance in the domestic health care. Also organizational and social-medical problems that prevent the introduction and development of perinatal palliative care in Russian practice were described and analyzed. Taking into account international and Russian experience, as well as the opinions of experts, the main organizational and methodological approaches to the development of a new direction of palliative care in Russia have been formulated.
