Analysis of High Technology Health Care Waiting Time for The Breast Cancer Cases

Background. Currently, the fight against cancer is a priority for the development of global health. This is due to the high prevalence of these diseases and their significant impact on the health indicators of the population and life expectancy. Surgical treatment is the main stage in the combined and / or complex treatment of breast cancer, which includes not only performing radical mastectomies, breast resections, but also many operations using high-tech treatment methods. Thus, today the availability of high-tech medical care for patients with breast cancer is an extremely important and urgent problem. In order to increase the availability of high-tech medical care to the population, the regulatory and legal framework for providing high-tech medical care is being improved every year. Materials and methods, the empirical study was conducted on the basis of 18 depth interviews with breast cancer patients of the Federal Cancer Institute who underwent surgical treatment in the Department of Oncology and Reconstructive Plastic Surgery of the breast and skin from March to April 2021. The results of the study. The theoretical aspects of providing high-tech medical care to cancer patients were studied, as a result of which it was found that the program of state guarantees of free medical care does not have standard waiting times for high-tech medical care. In almost all 18 cases, the stages and order were violated based on the results of an in-depth interview. The deadlines for performing morphological verification of the disease and obtaining histological results in almost all cases corresponded to the standard ones, but there are delays for making an appointment with an oncologist in order to receive initial consultation and undergo various diagnostic methods of research. According to the majority of patients who took part in the in-depth interview, these factors are the reason for the increase in the duration of the start of specialized treatment. Due to the lack of regulatory waiting times for high-tech medical care in the program of state guarantees of free medical care, it was impossible to conduct an adequate comparative analysis of the existing deadlines. Practically, in all cases, the waiting time did not exceed 10 days from the date of submission of documents to the executive authority of the subject of the Russian Federation in the field of healthcare, which corresponded to the order of the Ministry of Health No. 824n of 02.10.2019. on "The procedure for organizing the provision of high-tech medical care using the unified state information system in the field of healthcare". Factors affecting the lengthening of waiting times for specialized and high-tech medical care are: insufficient awareness and cancer awareness are the reason for late access to an oncologist and, accordingly, an increase in time delay, a long queue for an appointment with an oncologist in a polyclinic, a shortage of oncologists, insufficient effectiveness of active detection of tumor formations and low coverage of the female population with dispensary examinations. Conclusion: Thus, the problem of analyzing the waiting time for planned medical care, especially for cancer patients, is currently extremely relevant. Regular monitoring of the compliance of the terms of various stages of cancer patients ' routing with the regulatory ones will allow timely identification of factors leading to the prolongation of the start of treatment of patients.
