Goal of research

To evaluate the possibilities of theimprovements of the system of detection and evaluation of occupational diseases (OD) and means of the social protection of citizens suffering from OD basing on the comparative analysis of the prevalence of the OD, legal framework and practices in Russia and abroad.

Methodology

Analytic review of the international literature, on-line survey and structured interview of physicians, poll of patients and employers; statistical analysis, analysis of the state statistics and legal acts.

Empirical base of research

Datafrom state and public health statistics, reports from the Federal social insurance foundation, data of on-line survey, physicians’ interview, poll’s data.

Results of research

Prevalence of the ODs and practices of their diagnosis and evaluation as well as means of social protection of citizens suffering from ODs were analyzed using data from Russia in comparison with foreign data.

Normative acts of Russian Federation, regulating different aspects of the medical care and social support, evaluation of ODs and rehabilitation of citizens suffering from ODs were analyzed. There are significant discrepancies in domestic regulation with the recommendations of the International Labor Organization, including the list of diseases recognized as occupational.

Significant organizational problems have been identified: there are discrepancies between the ODs statistics from various departments; organizations conducting medical and social expertise are not in a unified methodology system, divorced from primary medical care.

The configuration of interests of the employees, employers, health care and expert organizations, working for patients with PDs, and the demand for and user evaluation of the range of means of social support have been explored.

The analysis indicates the presence of systemic problems in the organization of the identification of OD and in the protection of citizens with them. Recommendations on the directions of necessary changes in this system are formulated.

Level of implementation, recommendations on implementation or outcomes of the implementation of the results

We formulated the recommendations which may be used for the:

  • improvements of the national normative acts, regulating the system of social protection of the workers exposed to the harmful environment, including the harmonization of national acts with the international agreements and guidelines from the special international organizations.;

  • improvements in the development of the methodology of the diagnostic and evaluation practices;

  • improvements in the network of the medical organizations participating in the diagnosis, evaluation of ODs, providing health care, social care and rehabilitation;

  • correction of the system of the means of the social support of citizens suffering from the ODs.
