Developing research-based approaches to independent health care quality assessment

Goal of research –  developing research-based approaches to independent health care quality assessment, building ratings of medical organizations

Empirical base of Research:

  • literature on independent health care quality assessment and ratings of medical organizations
  • current statistical forms and health care quality indicators for various types of providers
  • in-depth interviews with representatives of the medical community

Results of Research

Based on an analysis of both international and Russian experiences in developing independent health care quality assessment, it is proposed that Russia: a) build ratings designed for mostly patients, b) evaluate only key areas of health providers performance with a limited set of indicators, c) simplify to the maximum possible degree a pattern of ratings development.

Developing ratings is not limited to the assessment of public opinion and implies the assessment of objective indicators of providers’ performance – clinical and organizational. Subjective perceptions of patients usually don’t adequately reflect this performance. Therefore, the suggested guidelines are based on two groups of indicators: 1) objective indicators from statistical forms available, 2) subjective indicators based on the outcomes of public opinion surveys. Two ratings are based on these indicators: 1) rating of comparative assessment of provider performance with the use of objective indicators – an aggregated rating of objective indicators, 2) rating of comparative assessment of providers performance with the use of the outcomes of public opinion surveys  - an aggregated rating of public opinion. These are developed in a parallel way and then an integral rating is formed.

A report “Methodological recommendations of developing ratings of medical organizations based on independent health care quality assessment” has been developed. The recommendations are based on the current statistical data and can be used in all regions of the country.

The report “Methodological recommendations of developing ratings of medical organizations based on independent health care quality assessment” has been provided to the Federal Ministry of Health and was partly used for developing corresponding regulations. These guidelines can be used in all regions of the country
